I hereby authorize 89.5 KVMR to initiate debit entries to my account indicated on the electronic form. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law.
This authorization is to remain in effect until 89.5 KVMR has received notification from me of its termination in such time and in such manner as to afford 89.5 KVMR a reasonable opportunity to act on it.
Furthermore, if any such electronic debit(s) should be returned by my financial institution as Non-Sufficient Funds ( NSF ), I authorize 89.5 KVMR to collect a returned item fee up to the amount required to process the return item by electronic debit from my account identified above.
The recurring EFT payment will be withdrawn from my bank account and will be updated there on or around the 20th of each month.
I understand and authorize all of the above as evidenced by my checking the box on the appropriate electronic form.
